This is a contract for modeling services between H.S. & G.S. Photography, 
hereafter referred to in this document as photographer, and 
_______________, hereafter referred to in this document as model.

Model agrees to provide these services as an independent contractor at 
the agreed rate and minimum hours per session, model will be paid for at 
least the minimum at the close of each session.  Bonuses, raises, or 
additional payments are at the discretion of photographer.

Modeling sessions begin when model is ready to be photographed or when 
on-site preparations for that session, as specified by photographer, 
begin.  These preparations include specific makeup or costuming for a 
session, arrangement of props, lighting, and background scenery, but do 
not include transit time or attendance to personal grooming or makeup.

Model will arrive within 15 minutes of the time scheduled with 
photographer for each session, prepared for that session, according to 
prior discussions with photographer.  If model has not arrived within 15 
minutes of agreed start time, or arrives unprepared, the session may be 
delayed, or may be canceled, without consideration for model.  If 
photographer cancels any session with less than 3 hours notice, model 
will be paid for minimum session.

Travel sessions, such session being defined as any that takes place 
further than 100 miles from photographer,  will be treated as 
exceptional contracts, and model paid according to a separate agreement 
between model and photographer.  Photographer is responsible for all 
arrangements and expenses relating to transportation, lodging, and 
meals.  Documentation for travel, such as identification and passports, 
and model's personal effects including makeup and medication are model's 
responsibility.

Model is expected to follow photographer's guidelines regarding rest, sun 
exposure, and comportment during travel sessions.  Failure to do so may 
void model's contract, and result in immediate termination.

Photographer is not responsible for changes to schedules caused by 
aircraft delays, weather, strikes or insurrections, or acts of god.  In 
such cases, all reasonable efforts will be made to accommodate model's needs.

All photographs are the property of photographer, except those taken for 
model and covered by a separate agreement.  Photographer agrees to 
provide model with such photographs and agreement, at model's request, 
for time and materials costs, or by other arrangement between model and 
photographer.

do hereby give H.S. & G.S. Photography, and parties designated by the 
photographer, including clients, purchasers, agencies, and periodicals 
the irrevocable right to use my name (and any fictional name) and 
photographs for sale and reproduction in any medium for the purposes of 
advertising, trade, display, exhibition, or editorial use.  I have read 
this agreement, fully understand its contents, and affirm that I am at 
least 18 years of age.
